SERVICE CALL-IN FORM
This submission form is received instantaneously and as soon as a Counselor is available you will receive a confirmation call. Services and appointments are made on first come first serve basis. The Family will be contacted directly after you to finalize and set appointments. We will do everything we can to accommodate the requested date and time, but a request is just that until your confirmation call is received. Please do not proceed with the obituary until you have received such confirmation.
*New Year's Day, Easter Sunday, Mother's Day, Father's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Christmas Day and after 12 Noon on Christmas Eve and New Year's Eve are considered Holidays and are subject to a holiday surcharge. Please plan ahead to avoid these dates/ times and avoid incurring these extra fees.
**Services scheduled to arrive after 2 pm on any day of the week are required to give a $250 refundable deposit to cover any additional fees that might occur.
1.The Lot Owner or Authorized Representative must come in person to sign the Interment Authorization, arrange service details, and pay applicable fees.
2. Funeral Directors must give the Cemetery 48 hours notice for interment orders and service arrangements must be finalized by the family at least 24 hours prior to the scheduled arrival time at the cemetery or a rush penalty fee of $250 will be applied.
3. Check with us before setting the day and time of the funeral service to avoid unnecessary scheduling problems. We shall do all we can to accommodate your requested time, but we shall not alter other previously scheduled funeral services.
4. Families requesting to view the completion of an interment must do so from a distance of 50 feet or more away.
5. Where applicable, the first burial will be placed in the lower portion of the Grave or Crypt.
6.Cremated remains must be retrievable. Our ground inurnment fee includes an urn vault (interior dimensions:14"x10"x6.5"). You must make alternate arrangements with the cemetery a minimum of 48 hours prior to your service if your urn will not fit this standard urn vault.
